我建了一個landingpage，但不知道怎麼用數據庫

我建了個 zip提交的landingpage,用的是一個模板，現在自己在機子上測試，把數據庫鏈接了下，但是貌似沒用，點擊submit按鈕，仍然沒有任何反應。原網址是http://localhost/freeoffer/index.htm 點擊後 是 http://localhost/freeoffer/index.htm#  請教懂php和mysql的高手

問題都解決了，但是上傳網站 ,打開後出現
